This form must to be completed by BOTH teams in the event of a fixture not being played owing to the fault of one or both teams, please give us much detail as possible.

Remember to update your Full Time result as postponed and select the reason from the drop down menu.

**Do not complete this form if the reason for Postponement is 'Pitch Conditions' - This should be notified within Full Time Giving the reason as 'Pitch Conditions' **
