Registration process and Secretary details

Existing Players from 15-16 season -
Send in the old id card with a new Photo paper clipped to the card.
Plus a team registration sheet.

NB - Please send in a team sheet with every registration not just the initial registration with bulk of cards.(attached is a copy)


New Signing on:
Please fill in a registration form and include 2 passport size photos and a copy of ID ( eg passport, birth certificate). Please do not send originals.A team sheet needs to also be completed every time you send in a batch of registrations.
One photo must be stuck on the form in the area for a photo and the other paper clipped to the form not stapled.



Lost ID Cards:
If you have lost cards from last season then a new registration form needs to be filled in with 2 photos .One photo must be stuck on the form in the area for a photo and the other paper clipped to the form not stapled.A team sheet needs to also be completed every time you send in a batch of registrations.
Each team needs a minimum amount of players registered by 16th August.

7 a side – 5 players
9 a side – 7 players
11 a side – 7 players

All registrations sent in by 31st August will be returned at the open meeting before the season starts any others will need to be accompanied by a SAE or they will need to be collected from myself.
Please ensure SAE are included with the correct postage otherwise I am unable to post ID cards.
