This Form is for Referee Use Only to report any unwanted 'abuse' before during and after a football fixtures

This form is solely for the use of referees to report those issues that are not deemed worthy of an FA Report but have the potential if allowed to continue to cause upset to you and other referees who come across these teams, in particular the newly qualified referees. It is NOT to be used for 'on field issues' as you have the tools in your box to report and deal with those. It is to create a picture of particular teams over the season and their progression through the age groups. Last season a referee ceased refereeing due to consistent abuse from a team. If this tool had been available we may have been able to spot a pattern earlier and nip it in the bud.
An Example is Ref 1 has some parents giving grief from BJFL United and does cause upset then this should be reported. If Referee 2 has the same team and again they get some verbals which cause upset then referee 2 should report it etc etc. Eventually when BJFL United hit the trigger of 4 reports then they will be observed. To use it just click on the link and fill it in, hit submit and you should get a copy for the records.
